PGD Checklist

All pharmacies and locums working in Forth Valley are encouraged to download and use the PGD Checklist to ensure they have completed all the relevant individual authorisations for the services that they deliver ensuring that they are legally covered to do so.  A copy of each individual authorisation should be faxed, e-mailed or sent to the Community Pharmacy Development Team- please ensure that you have completed the correct version and that you have signed and dated it.  Contact details for faxing. e-mailing and posting can be found on the Contacts page.
