Patient Group Direction (PGDs)
All pharmacists and locums working in Forth Valley are encouraged to download and use the PGD Checklist to complete all relevant individual authorisations.
Contractors should ensure that all pharmacists and locums they employ are aware of the services you are contractually obliged to provide. Contracted services must be provided during all opening hours. Pharmacists and locums must be legally covered to provide services by completing the mandatory training required as indicated in the PGD and signing the individual authorisation.
A copy of each individual authorisation should be faxed, e-mailed or posted to the Community Pharmacy Development Team. Please ensure that you have completed the correct version and that you have signed and dated it. Contact details for faxing, e-mail and posting can be found on the Contacts page.
