Following on from the successful pilot carried out by NHS GGC where patients were discharged safely and timely directly from GRI to primary care, with community pharmacies supplying discharge medication and performing medicines reconciliation during patient review. NHS Forth Valley plan to test the concept with an initial supply only model where patients can be discharged from A31in FVRH or Thistle Suite in Stirling Community Hospital and receive their required medication from their community pharmacy.
The service aims to provide better patient flow from acute to primary care.
- By allowing patients to leave hospital when they are medically fit rather than waiting on discharge medication should improve patient experience
- The service aims to increase the number of patients discharged before 12 noon allowing time for them to go to their nominated community pharmacy.
- An earlier discharge should help reduce cumulative bed days.
Community Pharmacies will be able to view the immediate discharge letter through the PCR and supply the required medication using the ‘Supply of Discharge Medication Directly from a Community Pharmacy’ PGD.
